Our Rates

There are several different insurances that we have contracts with:

Medicare / Medi-Cal (Medicaid):

Fillmore Convalescent Center is certified by the California Department of Health Services (DHS) as a Medicare and Medi-Cal (Medicaid) provider, therefore we are authorized to accept these forms of payment:

HMO's:

We have contracts with several HMO's in Ventura County to provide services to their members. If your HMO is not part of our list we have great success in getting individual contracts approved by the HMO.

Presently we have contracts with:

Private Pay:

Our current private room and board* fees have been in effect since February 1, 2010. Fees for service are based on daily charges:

Respite Care:

These services are for short-term (maximum 14 days) private pay residents when the family needs a rest or to take a vacation.

Returned check Policy:

The return of a check issued to Fillmore Convalescent Center will result in a $25.00 service charge per check placed against the account of the resident on whose behalf the check was presented. Each account will be allowed two (2) returned checks after which payment by check will not be accepted. All returned checks will be forwarded to the prosecutor's office if the amount of the check plus the service fee have not been paid within (10) business days of the non-payment.

**Non-ACH fee is a monthly processing charge for those who elect to pay the regular monthly charges in a matter other than by Authomated Clearing House (ACH) through Fillmore Convalescent Center .
